Position Description

Great opportunity in Sacramento, CA for a Nurse Practitioner seeing Post Transplant Care patients working for a private group. 

  • Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)
  • Minimum 2 yrs exp as NP preferably with transplant, nephrology, or complex chronic disease populations
  • 40 hours a week - Mon-Fri 8a-5p
  • Duties will include working in a clinic 3 days a week and 2 days a week doing comprehensive health work via telehealth for lab review and follow-ups
  • Rare visits to home - maybe once a month if that would be required when necessary
  • Clinic flow - patients get labs drawn in clinic, then see NP, then NP reviews labs 48 hours later, and then follows up with patients and nephrologists
  • Program aims to proactively manage care - many patients have disengaged from nephrology follow-up despite being high risk
  • Must have active OR medical and drivers license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp
  • Salary based on experience $145-160K and includes plenty of great benefits
