Location: La Mesa, CA

Salary: $75.00 USD Hourly - $85.00 USD Hourly

Description: Our client is currently seeking a Nurse Practitioner or Physician Assistant

Hours:

*This is a part-time position of 3 days per week, 8-hour days (24 hours per week) *

Options:

1. 3 days a week Mon, Wed, Fri; 8-hour days: 24-29 hours2. 3 days a week Tues, Thurs, Fri; 8-hour days: 24 hours3. 3 days a week Mon-Fri dep on business need: 24-29 hours flexible with scheduling 

Responsibilities include:

  • Perform Wellness Visits in partner primary care clinics and facilities in the Chula Vista community. 
  • Drive to designated facility locations in your assigned geographic area.
  • Review patient medical and family history.
  • Review of current providers and prescriptions.
  • Perform height, weight, blood pressure, and other routine measurements.
  • Deliver personalized health advice.
  • Review risk factors and potential treatment options.
  • Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
  • Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
  • Provide referrals to appropriate medical resources. 
  • Utilize our EMR system to document patient visit/interaction accurately and timely.
  • Knowledge of HCC coding.
  • Other duties as assigned. 

Qualifications and experience:

  • Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
  • Current, unrestricted NP or PA license to practice in California, including DEA license.
  • Minimum of 2 years of experience in primary care, hospice, or urgent care/outpatient setting.
  • Minimum of 1 year of previous experience utilizing an EMR system.
  • Good working knowledge of HCC coding.
  • Strong computer skills including MS Office and Outlook is strongly desired.
  • Great verbal and written communication skills
  • Current drivers’ license, auto insurance and reliable transportation
